LEGAL NOTICE OF PUBLIC HEARING
AND OPPORTUNITY FOR WRITTEN COMMENT

Public hearings are held by the Illinois Department of Public Health pursuant to the state’s Health Facilities Planning Act. They are open to the public and afford interested parties an opportunity to present written and/or verbal comments relevant to a particular project. All allegations or assertions should be relevant to the need for a proposed project and be supported with two copies of documentation or materials that are preferably printed or typed on letter sized paper (8 1/2" x 11").

14-043 St. Elizabeth’s Hospital, O’Fallon

The Illinois Health Facilities and Services Review Board (“State Board”) is giving notice of a 2nd public hearing for Project #14-043 St. Elizabeth Hospital for the discontinuation of a 303 bed acute care hospital located at 211 South Third Street, Belleville, Illinois and the establishment of a 144 bed acute care hospital to be located at the intersection of Interstate 64 and North Green Mount Road, O’Fallon, Illinois at a cost of $253,471,535.

The public hearing will be held on Tuesday, November 18, 2014 at the Belleville West High School - Commons/Cafeteria, 4063 Frank Scott Parkway West, Belleville Illinois. Sign in to provide public testimony will be from 4:30 - 5:00 p.m. The public hearing will begin at 5:00 p.m. and will conclude at 7: 00 p.m.

The Public Hearing will be conducted by staff of the Health Facilities and Services Review Board pursuant to the Illinois Health Facilities Planning Act. The hearing is open to the public and will afford an opportunity for parties with interest to present written and/or verbal comment relevant to the project. All allegations or assertions should be relevant to the need for the proposed project and be supported with two copies of documentation or materials that are printed or typed on paper, size 8 ˝ “ by 11”. Consideration by the State Board has been tentatively scheduled for the December 16, 2014, State Board Meeting.

Comments should be sent to Mike Constantino at the address below. The State Board will post its findings in a State Board Staff Report, and the report will be made available via the internet on December 2, 2014. The public may submit written responses in support of or in opposition to the findings of the Illinois Health Facilities and Services Review Board.
