LEGAL NOTICE OF PUBLIC HEARING
AND OPPORTUNITY FOR WRITTEN COMMENT

Public hearings are held by the Illinois Department of Public Health pursuant to the state’s Health Facilities Planning Act. They are open to the public and afford interested parties an opportunity to present written and/or verbal comments relevant to a particular project. All allegations or assertions should be relevant to the need for a proposed project and be supported with two copies of documentation or materials that are preferably printed or typed on letter sized paper (8 1/2" x 11").

In accordance with the requirements of the Illinois Health Facilities Planning Act, Notice is given of a receipt of an application (#E-008-16 - Franciscan St. James Health-Chicago Heights) to discontinue a three hundred twelve (312) bed acute care hospital located at 1423 Chicago Road, Chicago Heights, Illinois. The application was submitted by Franciscan Alliance, Inc., and Franciscan Alliance, Inc. d/b/a Franciscan St. James Health-Chicago Heights, 1615 Dragoon Trail, Mishawaka, Indiana.

This exemption application for the discontinuation of an acute care hospital was declared complete on January 13, 2016. Consideration by the State Board has been tentatively scheduled for the March 29, 2016, State Board Meeting. Any person wanting to submit written comments on this project must submit these comments by March 9, 2016. The State Board will post its findings in a State Board Staff Report, and the report will be made available via the internet on March 15, 2016. The public may submit written responses in support of or in opposition to the findings of the Illinois Health Facilities and Services Review Board. The public will have until 9:00 am, March 21, 2016 to respond. The internet address used to access the report is:

A public hearing will take place pursuant to 20 ILCS 3960. The hearing is scheduled for Tuesday, February 9, 2016, from 3:30 p.m. to 9:00 p.m. with a one (1) hour intermission from 5:30 p.m. to 6:30 p.m. It will be held at the Bloom Township High School Auditorium, 101 West 10th Street, Chicago Heights, Illinois.

The Public Hearing will be conducted by staff of the Health Facilities and Services Review Board pursuant to the Illinois Health Facilities Planning Act. The hearing is open to the public and will afford an opportunity for parties with interest to present written and/or verbal comment relevant to the project. 16-001 Palos Community Hospital, Orland Park

In accordance with the requirements of the Illinois Health Facilities Planning Act, notice is given of receipt to modernize/expand an existing Medical Office Building (MOB) in Orland Park. Project 16-001, Palos Community Hospital, Orland Park. Applicants: The St. George Corporation, and Palos Community Hospital. The applicants propose to modernize and expand its existing MOB at its South Campus Outpatient Complex, located at 15300 West Avenue, Orland Park. The project will involve 248,760 GSF of newly constructed space, and 47,550 GSF of modernized space. Project cost: $133,202,791.

This application to modernize/expand an existing Medical Office Building (MOB) was declared complete on January 4, 2016.
