LEGAL NOTICE OF PUBLIC HEARING
AND OPPORTUNITY FOR WRITTEN COMMENT

Public hearings are held by the Illinois Department of Public Health pursuant to the state’s Health Facilities Planning Act. They are open to the public and afford interested parties an opportunity to present written and/or verbal comments relevant to a particular project. All allegations or assertions should be relevant to the need for a proposed project and be supported with two copies of documentation or materials that are preferably printed or typed on letter sized paper (8 1/2" x 11").

14-008 Neighbors Rehabilitation Center, Byron

In accordance with the requirements of the Illinois Health Facilities Planning Act, notice is given of receipt to modernize and expand an existing Long Term Care (LTC) facility. Project 14-008, Neighbors Rehabilitation Center, Byron. Applicants: Neighbors Rehabilitation Center, LLC, and Neighbors Property, LLC. The applicants propose to modernize the existing 101-bed facility, to include the addition of 30 LTC beds. The facility is located at 811 West 2nd Street, Byron. Project cost: $9,018,858.

A public hearing will take place pursuant to Part 1130.910. The hearing is scheduled for Monday, March 31, 2014 at 10:00 A.M. and sign-in for the hearing will be conducted from 9:30 A.M. 10:00 A.M. The hearing will be held at the Byron City Hall, Community Room, located at 232 W 2nd Street, Byron IL 61010.

The Public Hearing will be conducted by staff of the Health Facilities and Services Review Board pursuant to the Illinois Health Facilities Planning Act. The hearing is open to the public and will afford an opportunity for parties with interest to present written and/or verbal comment relevant to the projects. All allegations or assertions should be relevant to the need for the proposed projects and be supported with two copies of documentation or materials that are printed or typed on paper, size 8 by 11. Consideration by the State Board has been tentatively scheduled for the June 3, 2014 State Board Meeting.
