LEGAL NOTICE OF PUBLIC HEARING
AND OPPORTUNITY FOR WRITTEN COMMENT

Public hearings are held by the Illinois Department of Public Health pursuant to the state’s Health Facilities Planning Act. They are open to the public and afford interested parties an opportunity to present written and/or verbal comments relevant to a particular project. All allegations or assertions should be relevant to the need for a proposed project and be supported with two copies of documentation or materials that are preferably printed or typed on letter sized paper (8 1/2" x 11").

14-043 St. Elizabeth’s Hospital, O’Fallon

In accordance with the requirements of the Illinois Health Facilities Planning Act, notice is given of receipt to establish an acute care hospital in O’Fallon, Illinois. The application contained a Safety Net Impact Statement, and was declared complete on August 26, 2014. Project 14-043, St. Elizabeth’s Hospital, O’Fallon. Applicants: Hospital Sisters Services, Inc., Hospital Sisters Health System, and St. Elizabeth’s Hospital of the Hospital Sisters of the Third Order of St. Francis. The applicants propose to discontinue a 303-bed acute care hospital, located at 211 South Third Street, Belleville, Illinois and establish a 144 bed hospital located at the intersection of Interstate 64 and North Green Mount Road, O’Fallon Illinois. Project cost: $253,471,535.

A public hearing will take place pursuant to 20 ILCS 3960. The hearing is scheduled for Thursday, October 2, 2014 at 4:00 P.M. and sign-in for the hearing will be conducted from 3:30 – 4:00 P.M. It will be held at Lindenwood University in the Auditorium, located at 2600 West Main Street, Belleville IL.

The Public Hearing will be conducted by staff of the Health Facilities and Services Review Board pursuant to the Illinois Health Facilities Planning Act. The hearing is open to the public and will afford an opportunity for parties with interest to present written and/or verbal comment relevant to the project. All allegations or assertions should be relevant to the need for the proposed project and be supported with two copies of documentation or materials that are printed or typed on paper, size 8 ˝ “ by 11”. Consideration by the State Board has been tentatively scheduled for the November 12, 2014, State Board Meeting.

14-044 St. Elizabeth’s Hospital-Ambulatory Care Center, O’Fallon

In accordance with the requirements of the Illinois Health Facilities Planning Act, notice is given of receipt to establish a Medical Office Building adjacent to a new acute care hospital in O’Fallon, Illinois. The application contained a Safety Net Impact Statement, and was declared complete on August 26, 2014. Project 14-044, St. Elizabeth’s Hospital-Ambulatory Care Center, O’Fallon. Applicants: Hospital Sisters Services, Inc., Hospital Sisters Health System, and St. Elizabeth’s Hospital of the Hospital Sisters of the Third Order of St. Francis. The applicants propose to establish selected hospital services in a newly established Medical Office Building, located in close proximity to the proposed new hospital at the intersection of Interstate 64 and North Green Mount Road, O’Fallon. Project cost: $34,243,594.

A public hearing will take place pursuant to 20 ILCS 3960. The hearing is scheduled for Thursday, October 2, 2014 at 6:30 P.M. and sign-in for the hearing will be conducted from 6:00 – 6:15 P.M. It will be held at Lindenwood University in the Auditorium, located at 2600 West Main Street, Belleville IL.

14-040 NorthPointe Health & Wellness Campus Free Standing Emergency Center, Roscoe

In accordance with the requirements of the Illinois Health Facilities Planning Act, notice is given of receipt to establish a Free Standing Emergency Center (FSEC). Project 14-040, NorthPointe Health & Wellness Campus Free Standing Emergency Center, Roscoe. Applicant: Beloit Health System, Inc. The applicant proposes to convert its existing Immediate Care Center to a Free Standing Emergency Center. The facility is located at 5605 East Rockton Road, Roscoe. Project cost: $1,442,398.

A public hearing will take place pursuant to 20 ILCS 3960. The hearing is scheduled for Wednesday, October 22, 2014 at 11:30 A.M. and sign-in for the hearing will be conducted from 11:00 – 11:30 A.M. It will be held at Roscoe Village Hall, Council Chambers, 10631 Main Street, Roscoe IL 61073.
